Not illustrated.1917 Excerpt: ... PATHOLOGIC PULMONARY CHANGES DURING ENDOTRACHEAL ANESTHESIA FOR EXPERIMENTAL LUNG SURGERY CONRAD GEORG, Jr., M.D. ANN ARBOR, MICH. The chief dangers to life encountered in experimental lung surgery are pneumothorax and infection of the pleura. Various apparatus have been devised at different times during the development of intrathoracic surgery to overcome the evil effects of pneumothorax. The earliest successful type of apparatus for this purpose was Sauerbruch's cabinet, in which the attempt was made to perform intrathoracic operations under the ordinary physiologic conditions of negative pressure. The results obtained by this method were very encouraging, but as the apparatus was quite complicated in its construction and operation it soon became displaced by other methods of differential pressure. Positive pressure apparatus were also tried with the object of raising the pressure of air within the lungs to such a point that they would not collapse when the chest was opened. It was soon found that this method produced serious disturbances in the circulation of the lungs and thereby considerably increased the mortality of intrathoracic operations. Willy Meyer employed a combination of positive and negative pressure with successful results. It was soon discovered that pneumothorax could be prevented by the use of an apparatus which would produce an artificial exchange of gases in the lungs during the period of the operation. Meltzer's insufflation apparatus may be regarded as a simple and safe method for inducing anesthesia for experimental lung surgery when the correct adjustment between the pressure required in the endotracheal tube to overcome pneumothorax and the capacity of the delicate lung tissue to withstand it has been secured.
